Endovitae ("Endovitae," "we," "us," or "our") operates this website (the "Site"). This Privacy Policy explains what information we collect through the Site, how we use it, and the choices you have. It applies to the Site only — see the HIPAA & Protected Health Information section below for how information you share with us as a patient is handled differently.

Information you provide directly — when you fill out a contact form, subscribe to our newsletter, or otherwise reach out through the Site, we may collect your name, email address, phone number, and the content of your message.

Information collected automatically — like most websites, we automatically collect certain technical information when you visit, including your IP address, browser and device type, referring pages, and how you interact with the Site. Information from third parties — if you book an appointment through our external scheduling platform, that platform collects and processes your information under its own privacy policy, not this one. We encourage you to review it separately.

HIPAA & Protected Health Information

This Privacy Policy governs information collected through the Site — it does not apply to Protected Health Information ("PHI") that Endovitae collects and uses in the course of providing your care as a patient. That information is governed by the Health Insurance Portability and Accountability Act (HIPAA) and is described in our Notice of Privacy Practices, which is provided to patients and available upon request.

Please do not submit medical history, symptoms, or other health information through the Site's contact form or email. Our contact form is intended for general questions only — appointments are booked through our secure scheduling platform, and any clinical information should be shared directly with our care team through your patient portal or during your visit.

Changes to this policy

We may update this Privacy Policy from time to time. Changes will be posted on this page with an updated effective date.
